KeystoneCare in Philadelphia is seeking a compassionate Licensed Practical Nurse (LPN) for private duty home care with an ALS patient. You will deliver skilled nursing care per physician orders, administer meds, and monitor the patient’s condition.

Formación

• Graduate of an accredited LPN program.
• Current LPN license in Pennsylvania.
• Minimum of one year experience in home healthcare or a similar clinical setting.
• Familiarity with Feeding Tubes, Assisted breathing Devices (Ventilators) and Hoyer Lift.
• CPR certification.
• Strong communication and interpersonal skills.
• Self-motivated with the ability to work independently.
• Reliable transportation and valid Driver's License.

Responsabilidades

• Provide skilled nursing care in accordance with physician orders, patient care plans, and agency policies.
• Administer medications and treatments as prescribed.
• Perform ongoing patient assessments and monitor for changes in condition.
• Ensure patient safety, comfort, and overall well-being throughout the shift.
• Communicate effectively with patients, families, and interdisciplinary care teams.
• Educate patients and families on care plans, equipment use, and safety measures.
• Maintain accurate and timely documentation in compliance with agency standards.
