Qualifications

• Current LPN licensure in Indiana.
• Current CPR certification.
• Excellent communication and interpersonal skills.

Responsibilities

• Oversees medication distribution per physician orders and maintains medication management documentation and resident safety.
• Creates personalized service plans tailored to individual care needs while promoting resident independence and dignity.
• Communicates with physicians, healthcare providers, and family to discuss residents' physical, mental, and social well-being.
• Contributes to quality assurance and performance improvement initiatives.
• Participates in ongoing learning through seminars, staff meetings, and other learning opportunities.
