Job summary

The Gardens at Cedarwood in Franklin, MA is hiring a Licensed Practical Nurse (LPN) for the 11:00 to 19:00 shift. You will provide high-quality, patient-centered care to our residents, administered medications, and participate in care planning with the healthcare team.

Qualifications

• Current LPN license in Massachusetts.
• 1+ year experience in long-term or acute care or acute care setting.
• BLS certification.
• IV therapy certification (or willingness to obtain).
• Ability to work varied shifts, including days, evenings, and weekends.

Responsibilities

• Provide high-quality, patient-centered care to residents, including administering medications, wound care, and vital signs.
• Work collaboratively with the interdisciplinary healthcare team to develop and implement individualized care plans.
• Monitor patient progress and report any changes or concerns to the healthcare team.
• Participate in quality improvement initiatives and contribute to the continuous development of our care models.
• Maintain accurate and up-to-date records of patient care and treatment.
• Provide education and support to patients and their families on various aspects of their care.
