Job summary

The Gardens at Cedarwood in Franklin, MA is seeking a dedicated LPN to join our nursing team. You will deliver high-quality resident care, administer medications, and support the well-being of our residents in a compassionate, patient-centered environment.

As part of a multidisciplinary team, you will implement individualized care plans, monitor patient conditions, and collaborate with Nursing Managers to promote safety, infection control, and exceptional outcomes for every resident we serve.

Qualifications

• Current LPN licensure in the state of Massachusetts.
• Strong communication and interpersonal skills.

Responsibilities

• Provide direct patient care, including administering medications, dressing wounds, and performing vital signs
• Develop and implement individualized care plans in collaboration with our interdisciplinary team
• Monitor and report any changes in patient condition to the Nursing Manager
• Contribute to a safe and supportive environment by adhering to infection control protocols and maintaining a clean and organized facility
• Collaborate with other healthcare professionals to ensure excellent patient outcomes
