Job summary

Allaire Health Services, LLC seeks a compassionate LPN to join the St Johnsbury Center for Living and Rehabilitation. You will provide direct patient care, including medications, wound care, and vital signs, while collaborating with a care team to develop individualized plans for residents.

You will conduct initial assessments, maintain accurate records, participate in rounds, and uphold safety, dignity, and respect. A strong fit for those who value teamwork and patient- centered care.

Qualifications

• Current LPN license in Vermont.
• High school diploma or equivalent; associate's or bachelor's in nursing preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.
• Strong attention to detail and ability to maintain accurate records.

Responsibilities

• Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring.
• Work closely with other healthcare professionals to develop and implement individualized care plans.
• Conduct initial patient assessments and maintain accurate and up-to-date records.
• Participate in interprofessional team rounds and maintain open communication with patients, families, and colleagues.
• Uphold the highest standards of patient safety, dignity, and respect.
• Perform other duties as assigned by the nursing leadership team.
