Job summary

Spring Oak Senior Living in Forked River, New Jersey, is seeking compassionate Licensed Practical Nurses (LPNs) to join the Wellness team on a part-time or per diem basis. Provide hands-on resident care, administer medications, and support health, safety, and well-being.

Shifts run 3:00pm-11:00pm, 11:00pm-7:00am, every other weekend, with a pay rate of $32.00 per hour. Benefits include health/dental/vision, 401(k) with match, PTO, and a supportive family‑like environment.

Qualifications

• Current and active Licensed Practical Nurse (LPN) license in New Jersey.
• Successful completion of an approved practical nursing program and required licensing examination.
• Ability to work collaboratively in a fast-paced healthcare environment.
• Strong communication, organizational, and documentation skills.
• Compassionate bedside manner and excellent attention to detail.
• Experience in assisted living, senior living, or long-term care is preferred.

Responsibilities

• Provide resident care in accordance with individualized service plans.
• Assist residents with activities of daily living as needed.
• Administer prescribed medications and accurately document all services provided.
• Monitor residents for changes in health, behavior, or functional status.
• Promptly communicate resident concerns or changes in condition to the RN and care team.
• Assist with developing and updating resident service plans and clinical documentation.
• Respond to resident emergencies, provide first aid, and arrange medical follow-up.
• Maintain accurate records in logs, service plans, and daily care documentation.
• Provide compassionate one-on-one support to residents as directed.
• Help maintain a safe, comfortable, and respectful environment.
• Support the RN with staff training, assignments, and other clinical responsibilities.
