Qualifications

• Current state licensure as Licensed Practical Nurse (LPN).

• Oversees medication distribution as stated on physician orders, ensuring strict adherence to Community medication management documentation and procedure, and demonstrating steadfast dedication to resident safety.
• Creates personalized service plans tailored to individual care needs while championing resident independence and dignity after thorough assessment.
• Cultivates communication between physicians, healthcare providers, and family, fostering an effective exchange of information pertaining to residents' physical, mental, and social well-being.
• Contributes toward quality assurance and performance improvement initiatives.
• Engages in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences.
