Job summary

Wolfe County Health & Rehabilitation Center in Campton, KY is seeking Licensed Practical Nurses (LPNs) to join our care team across shifts. The position offers a revised pay scale with night and weekend differentials and experience-based pay.

As an LPN, you will administer medications, coordinate with the healthcare team, and support residents' daily living activities while maintaining high standards of safety and compassion.

Qualifications

• Must be a Licensed Practical Nurse with active license.
• High school diploma or equivalent minimum.
• Experience in long-term care preferred.
• Ability to work various shifts.

Responsibilities

• Administer medications accurately and document patient responses.
• Maintain effective communication among patient, family, staff and physicians.
• Perform delegated nursing functions per policies and procedures.
• Coordinate with health care team to ensure quality patient care.
• Provide patient and family teaching and support as needed.
• Complete duties with empathy, confidentiality and professionalism.
