You will develop personalized service plans, support resident independence, and coordinate with physicians, healthcare providers, and families to discuss residents’ well-being. Ongoing learning and QA participation are encouraged.

Qualifications

• Current state licensure as Licensed Practical Nurse (LPN).
• Current CPR certification.
• Exceptional communication and interpersonal skills.
• Must demonstrate the ability to serve with love, compassion, and dignity.

Responsabilités

• Oversees medication distribution per physician orders and adheres to Community medication management documentation and procedures.
• Creates personalized service plans tailored to individual care needs while championing resident independence and dignity.
• Facilitates communication between physicians, healthcare providers, and family regarding residents' physical, mental, and social well‑being.
• Contributes toward quality assurance and performance improvement initiatives.
• Engages in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences.
