Job summary

Sevita is seeking a Licensed Practical Nurse/Licensed Vocational Nurse to provide prescribed medical treatment and personal care under RN or physician supervision. The role emphasizes medication administration, health assessments, and coordination with external agencies to ensure consistent patient care.

The position requires an Associate degree in nursing, an active state license, and prior experience; travel is required as needed. Compensation starts at $32 per hour with competitive benefits.

Qualifications

• Associates degree in nursing or state certificate.
• One year experience working with population served in a nursing capacity.
• Current LPN/LVN licensure for state of residence; CPR/FA, HIV/AIDS certification, BBP, ACT training as required by state regulations.

Responsibilities

• Provides basic care and medical treatment under RN or physician direction.
• Completes health assessments and inputs health diagnoses into care plans.
• Monitors and records medications; liaises with pharmacy; administers medications as required.
• Oversees staff who administer medication and conducts QA checks on records.
• Provides emergency coverage and coordinates with external agencies as needed.
• Maintains confidentiality and upholds health care policies; follows universal precautions.
