Linden Ponds by Erickson Senior Living is actively seeking a Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) to provide compassionate clinical care to residents in our dedicated neighborhoods. Responsibilities include medication administration, care plan updates, and collaborative care with RNs and staff.

The role offers day, evening, and night shifts with hourly compensation ranging from $34 to $45, plus differential incentives.

Qualifications

• Minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.
• Licensed by State as a Licensed Practical Nurse (LPN).
• CPR certification required.
• Passion to care for seniors and be a strong resident advocate.
• Experience with electronic medical records is preferred.

Responsibilities

• Develop relationships with residents and families using a consistent caregiver approach.
• Assess, observe, and evaluate residents' clinical condition and document in EMR.
• Update or modify care plans as needed to reflect changes.
• Supervise and lead care associates.
• Administer medications timely and accurately according to orders.
• Participate in meetings related to resident outcomes and care.
• Respond to residents, families, or responsible parties in a timely, professional manner.
• Participate in reviews of clinical indicators (falls, wounds, nutrition, antipsychotic use).
• Perform other duties as assigned by the RN clinical leader.
