Job summary

Harmony Suites Rehabilitation and Wellness Center seeks a motivated LPN to join our post-acute care team in Maryland. The LPN will carry out nursing processes, collaborate with physicians and the multidisciplinary team, and support residents and families with physical and psychological care.

The role involves supervising staff, monitoring residents, administering medications, and maintaining thorough documentation while upholding infection-control standards and confidentiality.

Qualifications

• Must possess and maintain a current state license as a professional LPN.
• Graduate of an approved college/school nursing program.
• Minimum one year experience in a Long Term Care environment preferred.

Responsibilities

• Identify resident care requirements by establishing personal rapport with residents.
• Monitor and chart health information, including medication administration and supervision of staff.
• Provide emotional, psychological and spiritual support to residents and families.
• Promote independence by teaching residents and families about conditions, medications and self-care.
• Ensure quality of care by meeting health outcomes and standards.
• Collaborate with multidisciplinary team to resolve resident problems.
• Maintain infection-control practices and safe medication handling.
• Document resident care services in charts/records and maintain confidentiality.
• Maintain cooperative relationships with health care teams through communication.
• Contribute to team effort by accomplishing related tasks as needed.
