Job summary

Central Queens Rehabilitation and Nursing Center is seeking a Licensed Practical Nurse (LPN) to provide comprehensive resident care in a long-term care setting. Qualifications

• 1 year(s) of experience in long-term care preferred.
• Must possess a current, unencumbered, active NY LPN license.
• Must possess the ability to make independent decisions when circumstances warrant such action.
• Must possess the ability to interact professionally with residents, families, staff, and the public.
• Must be knowledgeable of nursing practices, laws, and guidelines for long-term care.
• Must possess leadership and supervisory ability.
• Must be willing to seek out new methods and incorporate them into nursing practices.
• Must relate information about resident conditions clearly.
• Must cope with mental and emotional stress of the position.
• Must possess basic computer skills.
• Must possess in-depth clinical and documentation skills.

Responsibilities

• Determines care quality by interpreting policies; enforcing NY state requirements and DOH regs.
• Completes rounds to ensure appropriate resident care.
• Provides shift reports to CNA staff and incoming nurses.
• Completes narcotic count with staff.
• Ensures residents have proper armbands.
• Records weights and vitals as required.
• Documents labs/diagnostics in Sigma; notes refusals.
• Reports condition changes to RN/charge nurse.
• Documents transfers, appointments, dialysis in Sigma.
• Manages Sigma dashboard for meds and treatments.
• Documents all meds and vaccine administrations; puts holds on unavailable meds.
• Documents topical treatments, trach care, suctioning, GT/PEG care.
• Documents all catheter care in Sigma.
• Documents IV care, calories, fluids in Sigma.
• Completes 24-hour reports and daily progress notes.
• Completes progress notes as needed.
• Maintains communication with residents and families; answers questions.
• Maintains communication with physicians for orders and info.
• Keeps administration informed of issues; participates in surveys.
