Job summary

LiveWell Homecare Agency, LLC seeks a compassionate Licensed Practical Nurse (LPN) to deliver direct resident care in a long-term care facility. You will administer medications, perform wound care, assist with ADLs, monitor residents' status, and tailor care plans with the interdisciplinary team.

You will ensure accurate documentation, coordinate admissions/transfers/discharges, and contribute to quality assurance, staff education, and regulatory readiness in a supportive, growth-oriented

Qualifications

• Active LPN/LVN license is required.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Familiarity with electronic health records and Microsoft Office.

Responsibilities

• Administer medications and provide wound care.
• Assist with activities of daily living (ADLs).
• Supervise nursing assistants and coordinate resident care.
• Monitor residents’ physical and emotional status; develop care plans with the interdisciplinary team.
• Ensure accurate clinical documentation and coordinate admissions, transfers, and discharges.
• Participate in quality assurance, staff education, and regulatory surveys.
