Centerville Place Assisted Living in Dayton, OH is seeking a licensed practical nurse (LPN) to provide direct nursing care to residents and supervise certified nursing assistants in accordance with regulations and facility policies.

The role emphasizes holistic care within the nursing model, medication administration, wound care, and coordination of admissions, discharges, transfers, and follow-up appointments.

Qualifications

• Long-term care experience preferred but not required.
• Nursing degree from an accredited college or university.
• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.
• Current CPR certification.
• IV certification, if applicable

Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current regulations and facility policies.
• Promotes holistic care for residents using the nursing model.
• Ensures policies and procedures are followed by nursing staff.
• Participates in admissions, discharges, and transfers as required.
• Assesses changes in residents’ status and notifies physician and family, documenting accordingly.
• Transcribes physician orders and carries out orders as written.
• Prepares and administers medications per orders and monitors for adverse effects.
• Performs wound treatments and documents changes.
• Schedules follow-up appointments and transportation as indicated.
• Provides nursing leadership to the unit staff.
• Other duties as assigned
