Job summary

New York Community Home Care in New Jersey is seeking a compassionate Licensed Practical Nurse (LPN) for our senior center. This full-time, in-person role focuses on patient visits, monitoring conditions, and collaborating with physicians to ensure quality care.

Responsibilities

• Perform patient visits and monitor conditions, conduct tests, and write reports for the physician.
• Address caregivers' concerns and adjust patients' in-home management plan.
• Test for muscle weakness, bedsores, and signs of infection.
• Listen to family concerns and answer questions.
• Educate caregivers and family on aftercare or ongoing care.
• Provide suggestions for improved healthcare to physicians and family members of the patient.
• Monitor patient recovery and compile reports for physician.
