Job summary

Amedisys is hiring a licensed practical nurse to deliver home health care in Lancaster County, Lancaster. This full-time day shift role offers competitive pay and the opportunity to support patients in their homes.

You will verify plans of care, collaborate with RNs and physicians, teach self-care, monitor vitals and manage medications during visits. Candidates must have an active LPN license and CPR certification.

Qualifications

• One year of clinical LPN experience.
• Current LPN license for the assigned state(s).
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
