Qualifications

• Current LPN licensure in Pennsylvania.
• High school diploma or equivalent; associate's or bachelor's degree in nursing or related field preferred.

Responsibilities

• Provide direct patient care, including administering medications, performing vital signs, and assisting with ADLs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Assist with wound care, tracheostomy care, and other specialized care as needed.
• Monitor and report changes in resident condition to the RN/LPN supervisor and other members of the healthcare team.
• Maintain accurate and up-to-date records of resident care and services.
• Collaborate with other departments to ensure a comprehensive approach to care.
• Participate in quality improvement initiatives and educational programs to enhance resident care and outcomes.
