Discovery Senior Living is seeking an experienced Licensed Practical Nurse (LPN) to provide compassionate, evidence-based nursing care for residents in a senior living community. You will support safety, dignity, and well-being while coordinating with interdisciplinary teams and families.

You must be capable of night and weekend shifts, work with electronic documentation, and contribute to quality care in a dynamic residential setting.

Qualifications

• Current LPN license in the relevant state or RN license as per state regulations.
• Experience in assisted living, long-term care, or skilled nursing is preferred.
• Strong clinical judgment and effective communication.

Responsibilities

• Provide nursing care to residents per individualized care plans.
• Monitor and document residents' physical and psychosocial status.
• Administer medications and treatments as prescribed.
• Coordinate care with physicians, families, and pharmacies.
• Support wellness programs and health promotion initiatives.
• Maintain accurate clinical documentation and care plans.
• Collaborate with caregivers and leadership to ensure quality care.
• Respond to emergencies with sound clinical judgment.
