Job summary

Maple Ridge Rehabilitation and Healthcare is seeking a Licensed Practical Nurse (LPN) to deliver high-quality nursing care in a short-stay rehabilitation and long-term care setting.

The LPN will collaborate with physicians, residents, and families, administering medications, document care, and participate in care planning and discharge processes. A PA license (LPN or GPN) and BCLS certification are required, with opportunities across shifts on-site.

Qualifications

• High school diploma or equivalent is required.
• Graduate of an accredited nursing program is required.
• Current American Heart Association BCLS training is required.
• Valid PA LPN/GPN license is required.
• Experience in a skilled nursing facility is preferred.

Responsibilities

• Directs, supervises, and assists nursing assistants and others in resident care.
• Promotes a cooperative environment to ensure continuity and quality of care.
• Admits residents and conducts initial care plans and assessments.
• Administers prescribed treatments and medications and documents them.
• Communicates effectively with residents, families, physicians, and staff.
• Participates in discharge planning and care plan meetings.

Education

High school diploma or equivalent

Graduate of an accredited school of nursing

Current AHA health care provider training (BCLS)

Valid PA state licensure as LPN or GPN

Experience in a skilled nursing facility
