Job summary

Maple Ridge Rehabiliation and Healthcare is seeking a Licensed Practical Nurse (LPN) to deliver compassionate care in a skilled nursing facility. The LPN will utilize the nursing process and collaborate with physicians and staff to meet resident needs.

Graduate LPNs/GPNs encouraged to apply with PA licensure, current BCLS, and SNF experience. Competitive benefits and on-site responsibilities include medication administration, care planning, and discharge support.

Qualifications

• High school diploma or equivalent.
• Graduate of an accredited school of nursing.
• Valid PA LPN or GPN license.
• Current AHA BCLS certification.
• Experience in a skilled nursing facility preferred.
• Ability to coordinate care and communicate with team.

Responsibilities

• Directs, supervises, and assists nursing assistants in providing resident care.
• Admits and assesses residents and participates in initial care planning.
• Administers treatments and medications as prescribed; ensures proper documentation.
• Documents changes in residents’ condition and reports to RN supervisor.
• Participates in care plan meetings and coordinates with other departments.
• Maintains infection control standards and charting per policy.
• Checks narcotics and maintains medication logs.
• Ensures safe and effective delivery of resident care during shifts.
