Job summary

Mind Healing & Wellness Center in Northborough, MA seeks a compassionate Licensed Practical Nurse (LPN) to join our outpatient mental health and substance use care team. You will support Nurse Practitioners in medication management, MAT, Spravato, and TMS workflows, with injections and patient follow-up.

You will coordinate care, monitor vitals and lab results, document in the EHR, and participate in quality improvement activities.

Qualifications

• Current and unrestricted Massachusetts LPN license.
• Graduate of an accredited practical nursing program.
• Current BLS/CPR certification.
• Strong clinical communication and organizational skills.
• Ability to recognize changes in a patient's condition and involve the NP.
• Comfortable working with patients with mental health and substance use disorders.
• Ability to manage multiple clinical responsibilities and maintain follow through.
• Ability to work collaboratively as part of an interdisciplinary team.
• Comfortable using an electronic health record.

Responsibilities

• Work closely with Nurse Practitioners in ongoing patient care and monitoring.
• Respond to medication questions and clinical concerns with NP guidance.
• Assist with medication ordering and coordination with pharmacies.
• Manage refill requests and prior authorizations.
• Monitor vital signs, labs, urine toxicology and follow-up tests.
• Administer and coordinate injectable medications per LPN scope.
• Coordinate Spravato and TMS caseloads, scheduling and follow-up.
• Maintain REMS documentation and program compliance.
• Document accurately in the electronic health record and participate in meetings.
