Job summary

Support Solutions Of The Mid-south Llc is seeking a Licensed Practical Nurse (LPN) to deliver patient-centered nursing care within our residential and clinical teams. The role emphasizes medication administration, vital signs, documentation, and coordination with physicians, nurses, and staff to ensure timely, quality care.

The ideal candidate holds a Tennessee LPN license, has at least two years of LPN experience, and demonstrates strong communication, organizational, and confidentiality skills

Qualifications

• Current Tennessee LPN license and proof of degree/diploma.
• Two years of LPN work experience preferred.
• Must meet state background checks and training requirements.
• Proficient in medical documentation and MAR reviews.
• Ability to work with multiple clinical teams and patients.

Responsibilities

• Administers medications and treatments per orders and policies.
• Obtains vital signs, medical histories, and health concerns for visits.
• Maintains accurate nursing documentation and MAR reviews.
• Coordinates care with RN, Psychiatric Nurse Practitioner, and others.
• Provides patient telephone support and handles follow-up coordination.
• Schedules and tracks medical, psychiatric, and therapy appointments.
• Responds to medical emergencies within the LPN scope of practice.
