Ambercare is looking for a dedicated Licensed Practical Nurse (LPN) to join our team in Albuquerque, New Mexico. In this full-time role, you will provide direct patient care, implement care plans, and educate patients and families. Ideal candidates will be graduates of accredited nursing programs with at least three years of nursing experience. Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Has three (3) years nursing experience, preferably in community/home health or medical/surgical.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.

Responsibilities

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education regarding self-care techniques.
• Coordinates home health services and communicates needs to the registered nurse.
