Progressive Directions, Inc. (PDI) in Maury County, TN is seeking a compassionate Licensed Practical Nurse (LPN) to join our nursing team.

You will provide care to adults with intellectual and developmental disabilities, implement care plans, administer medications, and monitor health needs to ensure safety and dignity. You will collaborate with physicians, therapists, and direct support staff to deliver person-centered care that aligns with DDA standards and PDI values.

Qualifications

• Current, active LPN license in Tennessee or Nursing Compact state
• Current BLS Certification
• Valid unrestricted driver's license
• Successful background check and drug screen
• Negative TB test

Responsibilities

• Provide nursing care as prescribed by physicians
• Implement and monitor health care plans and crisis plans
• Assist with daily living activities to promote comfort and independence
• Administer medications safely and document administration
• Record vital signs, seizures, intake/output, and other clinical data
• Maintain medical supplies and equipment
• Participate in ISP and Circle of Support meetings
• Accompany individuals to medical appointments and coordinate follow-up
• Review MARs and report variances promptly
• Support therapy plans and ensure device safety
• Monitor health trends and report concerns to DON
• Train or mentor new staff on health plans and documentation
