Job summary

Progressive Directions, Inc. (PDI) is hiring a compassionate Licensed Practical Nurse (LPN) to join our nursing team in Clarksville, TN. You will provide care for adults with intellectual and developmental disabilities, administering medications and monitoring health while ensuring safety and dignity.

You will collaborate with physicians, therapists, and direct support staff to implement care plans, document MARs, and support health outcomes under DDA and PDI standards.

Qualifications

• Active LPN license in good standing (TN or Nursing Compact state)
• Current BLS certification
• Valid driver’s license & reliable transportation
• Satisfactory background check and drug test
• Negative TB test on request

Responsibilities

• Provide compassionate nursing care as prescribed by physicians
• Implement and monitor Individualized Health Care Plans and Medical Crisis Plans
• Assist with daily living activities to promote comfort and independence
• Administer medications safely and maintain MARs
• Document vital signs, seizures, intake/output, and other clinical data
• Maintain medical supplies and equipment in clean condition
• Participate in ISP and Circle of Support meetings to coordinate care
• Escort individuals to medical appointments and ensure follow-up on referrals/tests
• Review/update MARs and report variances promptly
• Support therapy plans and ensure assistive devices are safe
• Oversee psychotropic medication documentation and consents
• Monitor health trends and report concerns to DON
• Train/mentor new staff on health plans and documentation
• Collaborate with physicians, therapists, and direct support staff for consistent care
