Qualifications

• LPN license required.
• CPR certification required.
• Two years nursing experience preferred.

Responsibilities

• Admissions, Discharge and Transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Follow infection control, emergencies, and other center policies.
• Collaborate with interdisciplinary team to provide optimal outcomes.
• Identify and report abnormal changes in residents' condition per center policy.
• Work under supervision per Nurse Practice Act and center policies.
• Deliver nursing care for long-term or rehabilitative patients.
• Collect patient data, make observations, and report pertinent information.
