WeCare Centers is seeking a compassionate Licensed Practical Nurse (LPN) to provide high-quality care in a long-term care setting. You will work under the supervision of a Registered Nurse and physician to monitor residents, administer medications, and maintain thorough clinical documentation.

Qualifications

• Active LPN license in good standing.
• Experience in long-term care preferred but not required.
• Strong clinical assessment and critical thinking.
• Excellent communication and teamwork abilities.
• CPR certification or ability to obtain.

Responsibilities

• Deliver high-quality nursing care under the direction of the RN and attending physician.
• Administer medications and treatments safely and accurately.
• Monitor residents' conditions and report changes promptly to healthcare providers.
• Complete accurate and timely clinical documentation.
• Assist with admissions, discharges, and resident assessments.
• Collaborate with physicians, nurses, therapists and other team members to develop care plans.
• Supervise and support CNAs while promoting teamwork and resident care.
• Ensure compliance with state/federal regulations, infection prevention practices, and facility policies.
• Foster a safe, respectful environment for residents and families.
