Responsibilities include direct patient care under RN/physician supervision, vital signs, daily living activities assistance, and accurate documentation. A compassionate, team-oriented mindset and NJ LPN licensure are required.

Qualifications

• Active LPN license in New Jersey.
• Minimum of one year recent experience as an LPN in a healthcare setting.
• Strong communication and interpersonal skills for interactions with patients and team.
• Commitment to compassionate, patient-centered care.
• Ability to collaborate as part of a healthcare team.
• Excellent organizational and time-management abilities.
• Familiarity with electronic health records (EHR) is a plus.
• Ability to assist patients with mobility as needed.

Responsibilities

• Provide direct patient care under supervision of an RN or physician, including administering medications and monitoring progress.
• Perform routine tasks such as taking vital signs and assisting with activities of daily living.
• Maintain accurate patient records of care and changes in condition.
• Collaborate with healthcare team to deliver high-quality patient-centered care.
• Follow applicable laws, regulations, and facility policies.
• Engage in ongoing education and training to stay current with practice standards.
• Assist with preparation and sterilization of medical equipment and supplies.
• Promote a safe, clean work environment by adhering to infection control protocols.
