Job summary
Outcome HC in New Jersey is seeking reliable Licensed Practical Nurses to join our team. You will provide daily patient care and support in a long-term care setting, with emphasis on accurate history, vital signs, and compassionate assistance.

Qualifications
- CPR/BLS certification required.
- Must hold an active LPN license in New Jersey.
Responsibilities
- Record patient medical history accurately.
- Take and record vitals (BP, temp, HR).
- Observe patient progress and side effects of medications.
- Monitor patient condition and chart fluid intake/output.
- Assist with daily activities (eating, bathing).
- Administer injections and prescribed medications; wound care as needed.
- Collect samples for lab tests.
- Provide emotional support and communicate with family.
