Job summary

Montgomery Nursing and Rehabilitation Center in the Hudson Valley area seeks licensed Practical Nurses (LPN) for full-time, part-time, and limited per diem positions to support short term rehab, sub-acute, and long term care. You will provide direct resident care, administer treatments and medications, monitor vitals, and collaborate with the care team to update the Nursing Care Plan.

Licensed and currently registered in New York as an LPN.

Work well under pressure & willing to work harmoniously with all staff.

Responsibilities

Provide daily support to unit leadership and respond to resident/family needs.
Use nursing process, judgment, & skills to set priorities necessary to deliver quality direct resident care.
Administer standard nursing treatments and/or medications & observes resident for desired or unwanted effects.
Communicate & collaborate with appropriate care team members to revise/update the Nursing Care Plan or findings.
Monitor & document vitals, treatments administered, & residents' responses.
Prepare, organize, oversee, monitor, & evaluate delivery of care for resident CNA assignments & activities.
