Qualifications

• Current Colorado LPN license or compact state license.
• Graduate of an accredited practical nursing program.
• Strong communication and clinical skills.
• Compassionate, dependable, and team-oriented.
• Long-term care experience is preferred but not required.
• Passion for providing exceptional resident-centered care.

Responsibilities

• Provide resident-centered nursing care.
• Administer medications and treatments in accordance with orders and scope of practice.
• Monitor resident conditions and promptly communicate changes to supervising staff.
• Collaborate with physicians, RNs, therapists, and other healthcare professionals to ensure quality outcomes.
• Maintain accurate and timely clinical documentation.
• Promote resident safety, dignity, comfort, and independence.
• Support and mentor CNAs.
• Ensure compliance with state/federal regulations and facility policies.
• Contribute to a positive, team-focused culture centered on resident care.
