Qualifications

• One-year acute care experience preferred.
• Current LPN license in Maryland, CPR certification noted but not required for Hospice.

Responsibilities

• Assist in gathering data to assess patients' physical, psychological, social, and spiritual needs.
• Collaborate with the registered nurse and interdisciplinary team in planning for needs at admission.
• Review care plans and provide input into updates.
• Provide direct patient care using clinical nursing skills and judgement.
• Educate patients and families and document outcomes in the medical record.
• Initiate emergency measures when patient condition dictates need.
• Prepare patients for tests and procedures.
• Administer and document medications according to policy and procedure.
• Delegate care to non-licensed personnel and report to the Registered nurse.
• Report to the Registered nurse with concise handoffs on patient condition changes.
