Job summary

Apploi is seeking a compassionate Private Duty Nurse to provide individualized home-based care for patients. The role requires a strong nursing background with experience in critical care and hospital settings, enabling high-quality medical care and family support.

Responsibilities include one-on-one patient care, collaboration with the primary nurse, skilled nursing tasks (tracheostomy, ostomy, wound care, IVs), vital signs monitoring, and assistance with ADLs, while maintaining infection

Qualifications

• RN license in the state of practice.
• 2 years of clinical field experience.
• Knowledge of medical records management and documentation standards.
• Strong skills in medication administration and patient assessment.
• Ability to perform essential nursing skills such as wound care, suture management, and specimen collection processing.
• Excellent communication skills with patients, families and healthcare teams.

Responsibilities

• Provide one-on-one private duty nursing care to an assigned patient, focusing on medical, personal, and daily care needs.
• Collaborate with primary nurse to assess, monitor and report changes in patient’s condition.
• Perform skilled nursing tasks including tracheostomy, ostomy care, suctioning, wound care, suture management, dressing changes, specimen collection and IV management.
• Monitor and document vital signs, intake/output, treatments, and patient care.
• Assist with ADLs such as feeding, personal care, ambulation, transfers and skin care.
• Educate and support patients and families on medications, treatments and discharge instructions.
• Accompany patients to appointments, tests, scans, or procedures as needed.
• Maintain a safe, clean, infection-controlled environment.
