Job summary

CenterWell Home Health is seeking a licensed practical nurse to provide one-on-one home-based care under the supervision of an RN. You’ll implement care plans, educate patients and families on disease management, and document visits and outcomes to support recovery.

You will collaborate with the patient, family and healthcare team, participate in care coordination and discharge planning, and travel to patients’ homes with a valid driver’s license and reliable transportation.

Qualifications

Current nursing license in the practicing state.
Two years experience as an LPN/LVN in a clinical setting.
Valid driver''s license, auto insurance and reliable transportation
Current CPR certification.

Responsibilities

Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
Implement individualized care plans in collaboration with the patient, family, and healthcare team.
Educate patients and their families on disease management, treatment options, and self-care techniques.
Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes.
Participate in care coordination activities and discharge planning.
