Job summary

CenterWell Home Health is seeking a compassionate LPN to provide personalized, one-on-one home care under the supervision of an RN. You will implement care plans, educate patients and families, and document visits and outcomes to support recovery at home.

In this role you will maintain clear communication with the care team, coordinate with physicians, and participate in discharge planning while ensuring safety and quality of care for our patients.

Qualifications

• Current nursing license in the practicing state.
• Valid driver's license and reliable transportation.
• CPR certification currently in effect.
• Two years experience as an LPN/LVN in a clinical setting.

Responsibilities

• Provide skilled nursing care to patients, onsite in their homes, under the direction of an RN.
• Implement individualized care plans with patient, family and healthcare team.
• Educate patients and families on disease management and self-care techniques.
• Maintain accurate records of patient care, including visits, interventions, and outcomes.
• Maintain timely communication with patients, physicians, clinical managers, and other healthcare team members; participate in care coordination and discharge planning.
