Job summary

A Place at Home is seeking a Home Health Aide to provide compassionate, in-home care, assisting clients with ADLs, meals, medications, and basic household tasks under an RN's supervision. You will offer companionship and help maintain a comfortable living environment, while following individualized care plans and HIPAA guidelines.

Applicants should have a state-certified Home Health Aide credential, strong communication skills, and reliable transportation.

Qualifications

• Completion of state-approved Home Health Aide certification course.
• Strong communication skills, both written and verbal.
• Compassionate and empathetic with a strong desire to improve the lives of others.
• Ability to work independently and maintain high levels of patience and understanding.
• Physically able to perform the duties of the position, including lifting and physical movement as necessary.
• Reliable transportation and valid driver's license.

Responsibilities

• Providing assistance with ADLs, such as bathing, dressing, grooming, and toileting.
• Assisting with meal planning and preparation, observing dietary restrictions when necessary.
• Assisting with medication reminders in compliance with established care plans.
• Performing light housekeeping tasks such as dusting, vacuuming, and laundry to maintain a clean and comfortable living environment for our clients.
• Assisting with mobility and transfers, ensuring client safety at all times.
• Providing companionship and fostering a nurturing relationship with our clients.
• Documenting and reporting changes in client's physical condition, behavior, or living conditions to the supervising nurse.
• Following HIPAA guidelines and maintaining the privacy of all client PHI (Personal Health Information).
• Participating in ongoing training and professional development opportunities.
