Job summary

Qualifications

• Ability to monitor vitals per plan of care
• Reliable transportation to patient homes
• Current CPR certification required

Responsibilities

• Work with the patient in their home to provide one-on-one care
• Assist with activities of daily living and personal care
• Light housekeeping
• Meal preparation
• Monitor vitals (temperature, pulse, respiration) according to plan of care
• Remind patient to take medications as prescribed
• Maintain records and communicate with family/physicians
