Job summary

Live Well Home Care is seeking caregivers to provide in-home personal care and support to clients in their own homes and communities in West Virginia. You will assist with activities of daily living, meal planning, housekeeping, and transportation while ensuring safety and compassionate service.

The role includes companionship, emotional support, and respite care for families, with duties guided by a written care plan and overseen by the Branch Manager.

Responsibilities

• Assist with activities of daily living: ambulation, bathing, dressing, exercise, feeding, hair care, nail care, skin care, toileting, transferring
• Safely supervise the home environment to ensure client’s safety and security
• Plan and assist with meal planning and preparation; perform routine housekeeping and laundry
• Provide companionship and social interactions; offer emotional reassurance and encourage cognitive activities
• Provide respite care for families per care plans and coordinate with Branch Manager
• Run essential shopping/errands and handle client funds as outlined in the care plan under supervision
• Assist with following written/diet plans; reinforce diet maintenance per care plans
• Escort clients to medical facilities, appointments, and outings as specified in care plans
• Assist clients with communication by writing or typing correspondence
• Collaborate with Client Care Team (Branch Manager and Regional Leadership) to provide input
• Ensure service delivery complies with policies and procedures; monitor supplies and resources
• Follow the written care plan; perform duties as assigned by Branch Manager
• Observe clients and environments and report unsafe conditions to Branch Manager
