Job summary

SYNERGY HomeCare is seeking a compassionate Home Caregiver to join our West Valley team in Phoenix, AZ. You will provide personalized, one-on-one care to clients in their homes, supporting daily living activities and promoting independence with safety in mind.

The role includes medication assistance, meal preparation, light housekeeping, and documentation of care. You will collaborate with other healthcare professionals to ensure seamless care and positive client outcomes.

Qualifications

• High school diploma or equivalent required.
• Ability to lift, bend, and stand for extended periods, with occasional lifting of 25 pounds or more.
• Reliable transportation and valid driver's license (if required).
• Ability to work varied shifts, including evenings, weekends, and holidays.

Responsibilities

• Provide one-on-one home care, assisting with activities of daily living such as bathing, dressing, and grooming.
• Assist with medication management, including administration and monitoring.
• Prepare healthy meals and snacks as needed.
• Help with light housekeeping, laundry, and other household tasks.
• Promote client independence while ensuring safety and well-being.
• Develop positive relationships with clients and families.
• Document all care provided and client progress.
• Collaborate with healthcare professionals to coordinate care.
