The Home Care Assistant provides in-home support to seniors and disabled clients under supervision of the RN Assessor and Home Care Supervisor in their assigned area.

Responsibilities include performing home care services per the care plan, documenting visits, and reporting scheduling changes to the local office. Must hold PCA or CNA credentials and maintain required background checks and training hours.

Formación

• Must be certified as a Personal Care Aide or Certified Nurse's Aide/Assistant.
• Must maintain automobile liability coverage as required by state law.
• Background checks including state/federal and maltreatment checks are required.
• Training for PCA certification may be waived with prior related experience.

Responsabilidades

• Performs in-home services as outlined by the care plan and aide assignment.
• Arrives at and departs from client homes as scheduled; document visits.
• Report schedule changes or absences to the local office immediately.
• Observe ethics and confidentiality per policy manuals.
• Attend mandated in-service trainings and complete required hours annually.
• Adhere to all In-Home Services procedures and policies.
