Job summary

Evolving Solution Services seeks a dedicated Home Health Aide to support patients at home, promoting independence and comfort. You will work with healthcare teams and families to implement care plans, monitor changes, and report promptly to ensure safety and well-being.

Responsibilities include assisting with daily activities, documenting care, and maintaining sanitary home environments while prioritizing patient dignity and compassionate communication.

Qualifications

• High school diploma or equivalent.
• Completion of a state-approved Home Health Aide training program.
• Certification as a Home Health Aide (HHA) or equivalent credential as required by the state.
• Basic knowledge of patient care and safety procedures.
• Ability to perform physical tasks such as lifting and assisting patients with mobility.

Responsibilities

• Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support.
• Monitor patients’ health status by observing physical and emotional conditions and reporting any changes to healthcare professionals.
• Provide medication reminders and assist with basic medical tasks as directed by nursing staff or care plans.
• Maintain a clean, safe, and sanitary environment in the patient’s home to prevent infections and promote comfort.
• Document care provided and patient progress accurately using computer data entry systems.
• Prioritize tasks effectively to manage time and meet the needs of multiple patients when applicable.
• Work independently with minimal supervision while adhering to established care protocols and safety standards.
