Job summary

Responsibilities

• Provide social work support to patients based on health status, diagnosis and patient/family needs.
• Advocate for patients; help navigate complex health systems and coordinate care.
• Collaborate with interdisciplinary teams to ensure post-acute care planning and resources.
• Refer patients to community resources and assist with access to needed services.
• Complete governmental assessments and level of care documentation.
• Assist indigent populations with resources to prevent readmissions and improve outcomes.
• Handle complaints promptly and escalate appropriately.
• Perform other duties as assigned.
