Job summary

Pathways Hospice & Transition Home Healthcare in Texas is seeking a PRN Social Worker to support hospice and home health patients in the North service area. You will conduct admissions assessments, coordinate social work services, and partner with the nursing team.

Requires a Bachelor’s or Master’s in Social Work and Texas licensure, plus at least two years of supervisory social work experience in hospice or home health.

Qualifications

• Bachelor’s or Master’s degree in Social Work; Texas licensed.
• Minimum 2 years supervisory social work experience in hospice or home health.
• Excellent organizational and communication skills; able to work with diverse stakeholders.
• Proficient with Word, Excel, and Electronic Health Record (EHR) systems.
• In good standing with Federal and State Government.

Responsibilities

• Plans, develops, organizes implements, evaluates, and direct the social services program of the hospice patient.
• Interviews resident and family during admission to complete a social history and assess patient needs.
• Explains and obtains Advanced Directives (Directive to Physician, Medical Power of Attorney).
• Works with Nursing to ensure Do Not Resuscitate orders are obtained.
• Develops and implements policies for identifying medical and psychosocial needs.
• Applies Social Work Theory with supervision to restore or enhance functioning.
• Other duties as assigned.
