Job summary
Regional Hospice seeks an experienced Social Worker (MSW preferred) to join our interdisciplinary team serving the Lebanon, MO area. The role supports patients in various settings and collaborates with families and caregivers to provide comprehensive hospice care.
The ideal candidate has a master’s degree in social work (MSW preferred) or a BA with relevant experience, and is comfortable with flexible hours and using EMR systems for documentation. Mileage reimbursement is provided.
Qualifications
- MSW preferred; Bachelor's in Social Work considered
- Hours primarily Monday–Friday 8am–5pm with crisis coverage
- Hourly rate with mileage reimbursement; compensation based on experience
- Computer skills for charting/EMR; basic Word, Excel, Email; smartphone for mileage apps
Responsibilities
- Visit patients wherever they reside (home, SNF, ALF, hospital).
- Complete psychosocial assessments and document in EMR.
- Utilize community resources for patients (in-home services, Meals on Wheels).
- Assist patients, families, and caregivers with death/dying issues and grief support as needed.
- Provide information, complete hospice admission paperwork, and educate on hospice services.
