Job summary

Qualifications

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop a bereavement plan of care addressing family caregiver needs.
• Educate patients and families on learning needs, barriers, and resources; document in EMR.
