Job summary

Advantage Home Health Services in Pottsville, PA seeks a Hospice On-Call RN – PRN to join our home-based care team. The RN conducts Start of Care assessments, drafts care plans, reconciles medications, and communicates with Case Managers to coordinate patient care in the home setting.

We value compassionate, independent nurses with strong clinical skills, travel capacity, and knowledge of hospice regulations.

Qualifications

• Active RN license in the state of practice (minimum 1 year preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Compassionate and empathetic approach to patient care.
• Ability to work independently and as part of a team.
• Knowledge of hospice regulations and standards of care.
• Reliable transportation and willingness to travel.

Responsibilities

• Conduct Start of Care comprehensive assessments.
• Draft care plans, reconciling patient medications, and ensuring orders for care are accurate and complete.
• Preparing accurate, orderly, and timely documentation.
• Coordinating care with patient and provide proper reports to Case Manager.
• Provides health care instructions to the patient/family as appropriate per assessment and plan.
