Agape Hospice Care in Powder Springs, GA is seeking a dedicated Registered Nurse to administer skilled nursing care for hospice patients in their homes. You will coordinate with the interdisciplinary team and family to ensure compassionate, high-quality care.

The role requires state RN license, CPR certification, and strong communication skills. You will perform assessments, develop and adjust care plans, provide education to families, and participate in on-call rotations as needed.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office experience preferred.

Responsabilités

• Initial and ongoing comprehensive assessments of patient needs including physical, functional, psychosocial, and environmental factors.
• Implement and revise the plan of care (POC) as needed.
• Educate patients and families about disease, self-care, end-of-life care, nutrition, and care coordination; provide training to staff.
• Initiate preventive and rehabilitative nursing procedures.
• Maintain progress notes and coordinate services with the IDT and family.
