Job summary

Agape Hospice Care seeks a compassionate Registered Nurse to provide skilled nursing care to patients in their homes across Powder Springs, Dallas, Hiram, Austell, and Mableton. You will coordinate with the interdisciplinary team and referring agencies to ensure seamless, patient-centered care.

This role requires a current RN license, CPR certification, and strong communication and organizational skills; hospice or home health experience is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal and communication skills.
• Demonstrate skill in nursing practice and ethics.

Responsibilities

• Initial and ongoing comprehensive assessments of patients' needs and ADLs.
• Implement the individualized POC and revisions to the plan.
• Educate patients and families on disease processes and end-of-life care.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical notes and document progress toward goals.
• Coordinate all patient/family services with the IDT.
• Inform physicians of changes and evaluate responses.
• Provide on-call coverage as assigned.
• Other duties as assigned.
