Job summary

Harmony Hospice is seeking an experienced Hospice RN in Chicopee, Massachusetts to plan, coordinate, and provide compassionate hospice care per the patient’s plan of care. You will collaborate with an interdisciplinary team to address physical, emotional, and spiritual needs of patients and families.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed in the State of Massachusetts.
• One year of RN experience; Hospice experience preferred.
• Excellent assessment skills and problem-solving skills.
• Strong organizational and time management skills.
• Ability to communicate well, both verbally and in writing.

Responsibilities

• Coordinate hospice care with team members, patients, and families.
• Evaluate the effectiveness of hospice services for patients and families.
• Utilize the nursing process to provide professional nursing care.
• Consult with attending physicians and Hospice Medical Directors about changes in patients' conditions.
• Coordinate patient services with IDG and contracted services as needed.
• Submit complete, accurate clinical notes in a timely manner.
• Participate in IDG and case conferences.
• Develop and implement the initial plan of care with IDG.
